Somerton Family Medical Associates provides primary care and internal medicine – including comprehensive physical and mental healthcare – to individuals aged 16 and over in Northeast Philadelphia and the surrounding suburbs.
Services Include:

- Same-day visits for illness

As a Redeemer Health physician practice, Somerton Family Medical has access to an entire range of healthcare and health-related services for every stage of life including a full-service hospital, home care and hospice services, adult nursing and retirement communities, and low income and transitional housing programs for those in need.
Patient Portal
At Somerton Family Medical, we want you to be empowered about your own health. That’s why we offer a Patient Portal – a secure place where you can view your test results and health records, as well as schedule appointments.
If you’re a current patient of Somerton Family Medical and have received your patient portal sign-in information, sign on now.
If you’re a new patient or haven’t received your Patient Portal logon information yet, be sure to get it at your next appointment.
Prescriptions
For prescription refills, please submit your request through the Patient Portal or call our office and follow the telephone prompts. Please allow at least 24 hours for the prescription to be called in. You’ll need the following information:
- Your full name
- Your date of birth
- Your phone number
- Name of medication and dosage (30- or 90-day supply)
- Pharmacy name and number (or mail order info)
Referrals
For referrals, please submit your request through the Patient Portal or call our office and follow the telephone prompts. Please allow at least 24 hours for the referral to be completed. You'll need the following information:
- Your full name
- Your date of birth
- Name of specialist
- Provider NPI number
- Phone number for specialist
- Patient diagnosis code
- Date/time of appointment
Patient Forms
Welcome to Somerton Family Medical. As a new patient, you will be required to complete some forms before your appointment. In order to make your first appointment go as quickly and smoothly as possible, please complete all forms and bring them with you to your first appointment along with your most recent insurance card, medical records from previous physicians, list of specialists you're currently seeing, list of medications you are taking, including herbal supplements (you can also bring the medications in to review), a photo ID and your co-payment. If you’re a new patient, please arrive 15 minutes before your scheduled appointment.
